The cold agglutinin syndrome is an uncommon disor-ders described more than one century ago. Red blood cellsare lysed by complement and undergo phagocytosis by thereticulum endothelial system at low temperatures.
We report the case of an 80 year old woman who suf-fered anemia and respiratory symptoms, treated with antibi-otics on ambulatory basis. Her laboratory data showed ahigh titer of IgM cold agglutinins (1:8192). This disordermay be idiopathic, but secondary forms are seen in patientswith Mycoplasma infection or malignant hematologicalneoplasm. We discuss this syndrome as well as its patho-physiology, clinical presentation, laboratory studies andtreatment.
