Análisis bacteriológico y bioquímico de líquido amniótico en pacientes con ruptura prematura de membranas

Resumen

n la Obstericia actual existe una unificaciúl de criterios en cuanto al manejo de la Ruptura Pretérmino de Membranas, y es el manejo conservador de la Ruptura Prematura de Membranas. El gran fin del manejo expectante de la RPM es disminuir la morbilidad y mortalidad perinatal, y nuestro departamento no es la excepción. El gran dilema de este manejo ha sido el constante riesgo de infección que yace sobre el binomio madre-feto. La corriente actual es la utilización de métodos de detección precoz de infección intrauterina, pero aún con cierta controversia.

     


En nuestro estudio, los grupos poblacionales incluídos han mostmdo en sus diferentes variables ser muy homogéneos. Es importante mencionar que en dos variables importantes estos grupos se dispersaron; uno fue la edad gestacional al momento de la interrupción y, consecuentemente, el peso neonatal, las cuales son justificadas por que el grupo control corresponde a un grupo de mayor edad gestacional asociada a diferente patología.

Varios autores han publicado acerca de la relación de diferentes factores de riesgo y la presencia de RPM ( 15 , 16 ). En nuestro estudio encontramos que de los factores más comunes asociados a RPM, según la literatura mundial, sólo el antecedente de coito durante el embarazo tuvo significancia estadistica. El antecedente de RPM previa en la paciente, aunque no significante estadisticamente, presentó un riesgo relativo de 1.88 con un valor de p = 0.07.
